LIPOSUCTION IN COSTA RICA

It is a procedure to extract fat by applying a vacuum or with a syringe to create the vacuum to model the figure. The recovery is quick and wounds are small.

I use local anesthesia and general anesthesia, I prefer local anesthesia because a band is placed on the patient once finished, and then sent home; the recovery period is easier for the patient.
When the procedure is done with intravenous sedation or general anesthesia, recovery on the first and maybe second day may be accompanied with nausea, which does not happen with pure local anesthesia. In both cases there will be somnolence, stronger with intravenous sedation or general anesthesia.


Local anesthesia lets patients move comfortably from one position to the other, which with other anesthesia is more difficult. In some cases local anesthesia permits the patient to stand up and have results reviewed before the procedure is finished.

Should all cases use local anesthesia?

No, each patient is different and each surgeon should work with what he feels comfortable and safe. But every patient is different and some may require general anesthesia. Smokers and drug addicts, for example, are very anxious persons, with low tolerance to frustration; it could be very uncomfortable to have them for many hours on a surgical table.
Every now and then we find patients that do not belong into these groups, but are best managed totally sedated.

The skin’s capacity to adjust to a new fat content is well typified here. Skin usually shrinks very well, even a few hours or days after the procedure. If the patient has stria or flaccidity, a limited result is to be expected. The liposuction is not a treatment for cellulite or obesity.

What contraindications do these surgeries have?

Any patient to be submitted to this kind of surgery should be in good health conditions. Patients with some (conditions) diseases may be operated provided they are under control and duly medicated; this includes patients with arterial hypertension, asthma, diabetes, anemia, thyroid disease.
Although menstruation is not a disease it has a notorious influence. Patients may be psychologically and physically more sensitive, and may affect results. My advice is, especially if working with local anesthesia, to avoid doing it 2 days before and after the menstrual period. Even with general anesthesia recovery is more uncomfortable.

Are results permanent or could I gain weight?

In theory, results are permanent. This means that even in adults of normal weight, the fat cells do not multiply, once we remove the fat cells from one place, their number is reduced. Remaining cells may accumulate fat to some extent.
This means we could ruin some of the results. For this reason, patients have to be careful afterwards. If they do they can enjoy the results for many years, even improve them.


How many liposuctions can be done in one same area?

Predictably, one. After it, there will be scar tissue that will make a uniform result more difficult, which worsens after a second, third, etc.

How many areas of the body may be treated?

We have treated most areas of the body where fat is deposited, from the face to the ankles. Some areas respond better than others; generally the most common areas are the abdomen, waist, hips, double chin and thighs.


Before Surgery

If you have a history of abnormal bleeding you should warn your physician. Avoid any medicine containing aspirin or vitamin E at least 3 weeks before and after the surgery, since they alter blood clotting. Stop smoking at least 4 weeks before and after sugery. For you security you will be ask to bring lab testing and heart evaluation before surgery.

Recovery Process

Usually, bruises are minimum or nonexistent. It can also vary between patients. The areas I see them happen most frequently are the lateral parts of the thighs or internal part of the knees.
The final results you may see in a couple of months until the swelling to disappear. Is very important you take care about your diet to keep the results.

La cirugía debe ser realizada paciente y meticulosamente, poniendo atención a cada detalle y siguiendo lo planeado. Utilizar la mejor calidad en materiales y equipos. Nada es dejado a la improvisación, todo es definido antes de la cirugía en conjunto con la paciente. El seguimiento después de la cirugía es definitivo. Nos aseguramos de dar a nuestras pacientes tantos controles como sean posibles.

El aspecto de la clínica es esencial y obviamente, contar con instrumental y equipos de la mejor calidad. La tecnología definitivamente es importante hoy en día, pero aun lo es mas la destreza, técnica y dedicación que el cirujano le de a su caso.

Que evalua una buena recuperación? Permite evaluar el cuidado y la atención que un cirujano da a sus pacientes durante la cirugía. Entre más simple, menos dolorosa y rápida sea la recuperación más cuidadoso fue el manejo que ese cirujano dio a los tejidos del paciente durante la cirugía. Verifique esto interrogando a otras pacientes mientras aguarda por su consulta en la sala de espera.

BREAST AUGMENTATION IN COSTA RICA

There are over 3 million women worldwide who have breast implants. Most of them are satisfied with their results, some are not. Why? In many cases, patients performing a breast augmentation make wrong choices, based on minimum information, impulse, comments based on scarce knowledge, etc.

The best opportunity of obtaining the best result is having a first operation as with any esthetic surgery. The questions you make and decisions taken before your first operation will greatly determine results. The more seriously you search for information, the better the decisions and greater the possibility of getting an optimal result, if you combine it with a good dose of common sense.

The only totally natural breast is a totally natural breast. A breast augmented with implants is not totally natural and you can’t expect it to be.

If the benefits surpass the disadvantages and risks of a breast augmentation are acceptable to you, this surgery gives you the possibility to improve or restore what can’t be done any other way, in a predictable and safe way.

The purpose of breast augmentation is improving the size and shape of the breast. To the extent that results satisfy your goals, you may have a positive self-image; these feelings will contribute you a more open and positive image before others. But the only predictable change is larger breasts. Positive psychological effects are common but not predictable.











Your surgeon can only work with what you have: tissues, skin, fat, mammary glands, and your expectations. The better your tissues are, the greater the chances of having a beautiful result.
The more realistic your expectations are, the greater the chances of satisfactory results. The better your communication with your surgeon, he may better present you the options.

There are many choices in this surgery. Which one is best? You are an individual, different from any other woman. Your tissues differ, your breasts are different, and your wishes and expectations are probably diverse. Would you buy a dress the same size and style another woman would buy or wear? The best option for your friend is probably not the best for you. There are no identical women; breasts of each one are different. Why would a surgeon perform the same type of surgery, use the same kind or size of implant?

There is no perfect procedure in plastic surgery.
Perfection is not an option- improvement is.

There is no medical procedure that is free of inconveniences or hazards, neither is a breast augmentation. Nevertheless, this is a safe procedure.

Can mammary implants cause cancer? NO
The risk of a woman developing breast cancer with or without implants is the same -10% during her lifetime. However, if your risk is high because your mother, grandmother or sisters have suffered it, why place an implant that could deter from its detection. If this is your case, you should reconsider it.

Can mammary implants cause self-immune diseases? No. Nevertheless, if you suffer one of these diseases (lupus erythematosus, scleroderma, rheumatoid arthritis, fibromyalgia, etc.) or have a family history of these disorders, why place a foreign object in your body that could affect the immune system and trigger development of any of them faster?


Is breast augmentation surgery safe? Yes, provided you use good judgment and make the right decisions.

To obtain good results we have to work as a team. A team is made up of the patient and surgeon; when there are problems, they are directly related to bad team decisions. Implants are almost never to blame.

A good surgeon will assist you, giving all the necessary information so that you make good decisions; but remember the choice is yours. You’re responsible of your decisions.












Am I being superficial?
Most of us do things others consider vain or superficial. Would you let others decide in your personal choices? No! Right? What matters is to make them responsibly, and remember, wanting to feel normal is O.K.; it is normal wanting to be all that you can be. Inform yourself, consult, analyze, take your time; do not act under the pressure of others, or impulse (couple, husband, physician or clinic, etc.)
Remember, you will live with the result the rest of your life.

Do implants have to be changed?
You will probably need to replaced your implants in 10-12 years too. They are not permanent. For a good follow up and safety you will also need the regular year breast check up by you doctor.

Is it recommended to make a loan to perform a surgery?
If the financial burden is not in the way of you fully enjoying your investment, it’s o.k. If other than that, reconsider it.

Plan your surgery thinking of the present, but also at least 5 to 10 years ahead. What you do now will be with you the rest of your life.

What size of implant is advisable for me?
The most important thing to bear in mind before an augmentation mammoplasty is what your body and tissues allow. Imagine that in pregnancy and lactation, a woman’s breasts increase by one or one and a half size –from A to B or C perhaps; even if it’s a physiological increase, breasts suffer damage. Now think with a non-physiological increase, the placement of implants. To increase the size by one or two measures, we usually require a 200 cc and 275 cc prosthesis. You were provided with only one pair of tissues. If they are damaged, there is no way to replace them. Furthermore, these tissues don’t improve daily. We grow older each day, like or not. If you go beyond what your body permits, perhaps not now but later you will have to pay the price, physically and economically. When stria appear, your skin will stretch and grow thinner; you’ll probably have to plan more surgeries in the future to lift up your breasts, which implies more risks and scars.

An implant’s number of cubic centimeters does not mean a certain breast size. The final size of the breast depends of the quantity of mammary tissue the patient has before the surgery, more than the implant’s size. This means that the same implant may render different results on different patients. On the other hand, if your tissues have stretched further, you will require a larger size to adequately fill what another woman, whose tissues are not as stretched, would require.

When is it enough?
If your goal is a better result in the long term- For a woman that has never been pregnant, the surgeon must estimate the amount of normal stretching she would experience during pregnancy, given the tissues characteristics of each patient’s breasts. When you select an implant that is too large for what your skin can bear, irreversible changes may occur, resulting in irreparable consequences.












No woman’s breasts are alike, and neither can any surgeon make two identical breasts.
If you are somewhat of age, think of a woman whose breasts were large when she was younger. How do they look now? Thus, the larger the breasts, the worse they will look in time. Now, repeat ten-fold: the larger the implant that I ask for, the worse it will look further on. An implant may be too large, somewhat over 300 cc; without a doubt, when over 350 cc. We are not dealing with magic; it’s simply a matter of logic and common sense. So, to get the best results in the long term we have to weigh what you desire against what your tissues allow.

What do my tissues allow me to have? This requires assessment of your breasts, consideration of your height and body size.
Selection of the implant’s size may not be done at random or to suit the taste of others.

Does this mean that no woman should have very large implants?
Not necessarily. Sometimes larger implants are required to fill into too enlarged tissues. Other women simply want larger breasts, and they certainly have the right to get what they want. But they have to know what will happen in time.

Perfection is Not an option. Surgeons can only make improvements. Fortunately, you will see this as perfection, when comparing to what you had before; but in plastic surgery there is no perfection even if you choose a perfectionist surgeon.


What kind of implant is better, rounded or anatomical?
Implants may be rounded or breast-shaped (anatomical). The latter may produce results that are more natural but are more demanding to place. If your surgeon is not or does not want to be thorough, it’s probably best to use a rounded implant. If the chosen implant is very large (over 350 cc), tissues will stretch and the anatomical implant will misadjust as time passes. In those cases a rounded implant would be best, because with so big breasts the person is not looking for a natural result.

What kind of implant is better: silicon gel or saline solution?

Silicon gel implants are more similar to natural breasts when touched; they are more predictable because the material holds its shape better, and it’s safe. Silicon has been used for decades by medical teams causing no problems. Nevertheless, no specific type of implant is best for every patient. What’s best for you depends on what you want, what your tissues allow you to have, and the benefits and disadvantages you are willing to accept.













What is better, an implant over or behind the muscle?
When an implant is placed over the muscle –retromammary- the implant is covered on its lower part by the mammary gland and over it just by skin and fat. When an implant is placed behind the muscle, the higher part is covered by the muscle but the low part is covered by the mammary gland only. The closer the implant is placed to the tissue it’s trying to shape, the more predictable and precise results are. Thus, when the implant is placed over the muscle its esthetical result is usually better. There will be better control of the space between the breasts, filling of its higher part and less chance that over time the implant is displaced due to muscle contraction, towards the bottom, external part. Notwithstanding, more important than all of this, is that tissues have the adequate thickness to cover the implant. If you’re too thin it may be better to place the implant behind the muscle rather than risking the implant to be visible under your skin. Also, its retromammary location might make mamographies difficult to interpret; and when silicon gel implants are used they are associated with greater frequency of developing a very strong scar around the implant, which physicians call capsular contracture.

Once again there is no perfect choice, so you will have to accept some disadvantages with each location.

What is the best location for a scar: underneath the breast, around the areole or by the armpit? The standard location is underneath the breast’s contour since it allows for better visibility and greater control of the more difficult areas of the surgery. Its main disadvantage is the presence of a scar underneath the breast’s contour.
The periareolar incision tends to heal very well, but to place the implant we have to cut through the glandular tissue, which could cause more sensitive alterations, and a greater frequency of capsular contraction. If you have a tendency to produce ugly scars, the most visible area, the breasts’ most important esthetically, may not be a good place to have a scar. Axillary incision is suitable for a smaller group of patients, since it’s used to treat breasts that are in quite good conditions. The scar is outside of the breast, which for some may be a very good reason. But, to have comparable results to the preceding two, it has to be assisted with an endoscopic surgery team.


RISKS

Every medical procedure requires you to take some risks to obtain certain benefits, besides those of anesthesia and surgery in general. The following are some of the complications that may result from any surgical procedure that places a foreign object in the body.

§ Bleeding
§ Hematomas
§ Liquid accumulation around the implant
§ Changes in the nipple’s sensibility
§ Infection
§ Scars, Stains or skin changes in coloration and contour
§ Probable asymmetry
§ Capsular contracture
















§ Allergic reactions
§ Possibility of late calcification around the implant.
§ Thin Skin with the possibility to feel implant borders
§ Need to withdraw the implants due to complications

A hematoma is an accumulation of blood around the implant. A hematoma is not a bruise. Hematomas may occur in 1-2 of every 100 cases. When large they must be (drained) extracted. In such cases the risk of capsular contracture is higher. If you have a history of abnormal bleeding you should warn your physician. Avoid any medicine containing aspirin or vitamin E at least 3 weeks before and after the surgery, since they alter blood clotting.
An implant can be torn due to injury or impact to the breast or by the normal aging process of the implant.

What is a capsular contraction? A capsule or sort of covering is formed around any object placed inside the human body. A capsular contraction happens when said capsule contracts or presses excessively on the implant, making it hard, sometimes displacing it or altering its shape.. The risk of suffering it is around 5%. Patients who develop it will experience it the first year after surgery, but a small percentage could develop it much later. If you develop a significant capsular contraction, treatment is to surgically remove most of the capsule and place a new implant. If you develop a second contraction, there are two options, not attempting another surgery or removing your implants.

Repeated operations on your breasts are one of the most common causes of severe problems.

How to avoid a capsular contraction?
There is currently no way to predict which patients may develop it and how to avoid it. Nevertheless, it would be sensible to bear in mind the different factors contributing in its development and avoiding them. They could include the following:

· Use textured implants
· Avoid to the maximum the implant’s contact with the mammary gland, which could include aspects such as:
-Avoid large implants. The bigger the implant the greater contact area with the implant, if the implant is behind the muscle.
-Place the implant behind the muscle if it’s a silicon gel one. There’s no difference when saline solution implants are used.
-Bear in mind that the periareolar via has this problem more frequently than the inframammary or axillary vias.
· Decrease to the maximum the implant’s contact with the blood, which contains anti-inflammatory substances that increase the immune response to the implant.














What activities may I do after the surgery?
This question has different answers; what matters most is that you follow your surgeon’s instructions.
There will be some that suggest not doing diverse movements for 2-3 or 4 weeks. There will be others that allow certain level of normal activity or almost normal 2-3 days after. This will usually depend on the surgical technique each surgeon employs. The more precise the technique employed the lesser the trauma, and recovery will be faster. A total or almost total recovery is possible in a lapse of 48 hours or somewhat higher in most of our patients.

After the surgery may I not use the bra again?
Breasts, with or without implants tend to fall. Patients with implants on their breasts will experience this more slowly than those without them, but they will nevertheless. So, it is recommended to wear a bra whenever possible; if you don’t want to, occasionally, it’s o.k.

Will my breasts and nipples be exactly alike?
No, even if most people would like a perfect result as product of an esthetic surgery, the shape of breasts and the nipples’ position may be slightly different -one side from the other. These differences are also evident before the surgery, but most patients are aware of size rather than shape differences or assimetry.

Will my nipples face to the front?
Perhaps, if your nipples are facing front. Breasts and nipples are normally positioned slightly facing out. The reason for this is that the thorax is geometrically cylindrical and breasts have a conical shape more or less. If we place a pair of cones on both sides of a cylinder, they will most likely look to the sides. It’s not natural that breasts face to the front, although it could happen in a patient.

Is breast-feeding capacity lost when having a breast augmentation?
No, in general. Nevertheless, it depends.
If the patient gets the proper implant for her size, even if the implant exerts pressure on the mammary gland, it will cause no major harm nor alter its breast-feeding capacity. If the implant is too large and the gland is compressed and thinned by the implant, it could loose its function, partly or totally.
Something similar happens with the breast’s sensitive function- the greater the implant the larger the space to place it, requires cutting more nerves, and consequently more sensitive loss. If the implant’s size is adequate, sensitive loss is minimum, or none, and in case it happens, is temporary, that is, -a few months.

CUIDADO CON LOS TRATAMIENTOS ESTETICOS

INFORMACION SOBRE TRATAMIENTOS ESTETICOS

1) Qué requisitos debería pedir -como cliente- a la sala de estética que me va a atender? (patentes, permisos, máquinas, productos, etc.)

-Revisar que la clínica y las maquinas están aprobadas por el ministerio de salud de costa rica.
-Revisar si el especialista que la va atender es certificado, sea fisioterapeuta o esteticista, ahora hay muchas personas que sin experiencia se dedican a estos tratamientos estéticos causando grandes daños a corto o mediano plazo.
-Ver la calidad de los productos que se aplican, marcas, reacciones alérgicas.

-Limpieza del lugar, de instrumentos, valorar y preguntar si esterilizan los instrumentos de faciales, esponjas, paños, otros aparatos donde pudiera haber contaminación peligrosa, el jabón no elimina todas las bacterias, además hay elementos cortantes o punzantes que se pueden contaminar con sangre o fluidos, como el caso de acné y sangre en los faciales, y cortes como en las depilaciones o preparaciones antes para depilación con laser.

-PREGUNTAR nombre de medicamentos que le van ha ser aplicados o inyectados, actualmente en el mercado hay muchos productos no aprobados por el ministerio que traen de otros países “en una maleta sin aprobación” y que son inyectados para aumento de glúteos, rellenos faciales y corporales, que no se reabsorben y que años después producen malformaciones irreversibles. Químicos inyectados y o tomados a los que llaman “naturales” para adelgazar y redicir medidas que producen quemaduras internas y de piel irreversibles y necrosis (Perdida de piel), daño hepático, renales, problemas cardiacos y la muerte. Investigar por varios medios los medicamentos y sus reacciones adversas antes de aplicarlos.

-Comprobar resultados a largo plazo, Lo NUEVO no es lo mejor, Es mejor saber que ha sido probado por muchos años en el cuerpo, y que no ha resultado negativo, No se deje engañar con la moda, o por que lo aplicaron a sus amigas, verifique que después de unos años los resultados positivos se mantienen. No se deje engañar de anuncios mágicos. la magia no existe.


2) ¿Qué requisitos debe cumplir un profesional en estética?
–Que sea certificado
–Para algunos tratamientos debe estar supervisado por un profesional medico.
-Que no realice tratamientos que estén fuera de su competencia o profesión. Algunos esteticistas se dedican a hacer mini- liposucciones, inyecciones para aumento de glúteos con medicamentos no certificados o de alto riesgo que tienen que ser aplicados por médicos.
-Que el lugar donde le realicen el procedimiento sea adecuado, no un garaje, o un consultorio clandestino, recuerde que es su vida y su cuerpo y usted tendrá que vivir con el resultado de su decisión el resto de la vida.




3) Para la atención de la piel ¿cuáles son los servicios más comunes que puedo encontrar en una estética?

-Exfoliación Corporal: Remoción de Células muertas de la piel que permitirán una renovación celular devolviendo tersura y vitalidad en la piel. Hay diferentes tipos de exfoliantes; Sales Marinas, Algas, Frutas, Arcilla, dependiendo el tipo de piel.

-Envolturas y termo activación para hidratación profunda de la piel.

-Dermoabracion, Tratamiento de renovación cutánea, realizado en sala de operaciones, que remueve las capas superficiales y media de la piel, para mejorar arrugas finas moderadas y cicatrices de acné. Procedimiento que debe ser realizado por un dermatólogo o Cirujano plástico.

-Microdermoabracion, Abrasión superficial de la piel, para tratamiento de arrugas finas. Lo puede hacer la esteticista con supervisión de especialista.

-Peelings: Consiste en la exfoliación de capas superficiales de la piel por medio de agentes químicos. Tales como acido tricloroacetico, acido glicolico, acido salicílico, antioxidantes como la vinoterapia y vitamina C. Hay que tener cuidado con su aplicación y principalmente con la preparación de la piel del paciente antes del tratamiento, para evitar manchas y cicatrices en la cara.

-Tratamientos de Laser y Luz Pulsada, usado para depilación, tratamiento de varices, y manchas. Es un tratamiento adecuado siempre y cuando se utilice adecuadamente por un especialista y con aparatos adecuados y de alta calidad. Hay una gran mejoría pero no existen resultados del 100 % de efectividad.
El riesgo puede ser alto si no se aplica adecuadamente, quemaduras en la piel, manchas y hasta perdida de piel se puede presentar.

-Faciales y Limpiezas faciales, Tratamientos de limpieza profunda, ozonoterapia para regeneración celular, alta frecuencia, foto estimulación para aumentar producción de colágeno y corrientes para tonificación facial, así como hidratación profunda con velos de colágeno, frutas, sueros y vitaminas para una nutrición adecuada d la piel.
